Job Description
We are currently seeking a Home Health Physical Therapist Per Visit to join our team. In this role, you will be responsible for providing physical therapy services to patients in their homes on a per-visit basis.
The ideal candidate will have a valid physical therapy license, strong clinical skills, and a passion for helping patients achieve their rehabilitation goals.
You will work closely with patients, their families, and other healthcare professionals to develop and implement individualized treatment plans.
